Are you using any kind of caching plugin on your site? That could be the cause of the issue. The Mobile Smart plugin is required to detect mobile browsers and serve the mobile theme accordingly. Alternatively, you can also use the Any Mobile Theme Switcher plugin instead of Mobile Smart, as some users have reported that it is more stable compared to Mobile Smart.

Unfortunately there is no easy solution to this. You would need to use an image instead of genericon for it to be displayed in IE9. Simple enough solution, but will quickly get very tedious if you use Genericons in lots of places.

The settings for Graphene and Graphene Mobile Neo are separate, to allow for maximum customisability between the two themes that server different types of devices. However, there are some similar settings between the two which you can copy over using the “Copy relevant settings” button in GM Neo’s options page.
For headers and colours, yes you would have to set those again.
For the menu, you can assign the same menu used in Graphene to be used by GM Neo as well. Just go to WP Admin > Appearance > Menus to do this.

There is no such file in Graphene, Graphene Mobile, or Graphene Mobile Neo. It could be a plugin that’s trying to load that file.

In WP Admin > Settings > Mobile Smart, make sure you uncheck the Enable theme switching for tablets (e.g. iPad): option. If that is already unchecked, try using the Any Mobile Theme Switcher instead of the theme’s bundled Mobile Smart.
