It’s possible that you have an incomplete installation of either Graphene or WordPress. Try these things, in this order:
1) Temporarily disable ALL plugins by using FTP to rename your plugins folder.
2) Reinstall Graphene.
3) Reinstall WordPress.
Hard to offer more assistance without seeing the site.

Try temporarily disabling the Jetpack plugin and seeing if that fixes the issue.

It look like that “Extended Categories” widget may be the issue. There really should not be “categories” in the permalink.

You use a lot of plugins. Chances are that two (or more) of them are conflicting. Try disabling them.
If you can’t login to disable plugins, then FTP to your site and temporarily rename the plugins folder. If that fixes your problem, then you know the issue is a plugin conflict.

I used GIMP to create the header image on my blog, BTW. For a free program, it is amazingly powerful.

Yes. But you will need to combine those images into a single image that is 960×198 pixels in size. You will need to use another program to do this–either Photoshop (good, but expensive) or GIMP (good, but free with limited/quirky support).
