how come sometimes the slider adds an image but other times it does not?
Second is on this page http://smoker2vaper.com/reviews/mixing/ Diamond Crystal Review has an image but Absinthe concentrate does not, if i set a featured image I see one but it also adds that image to the header.

Enable the "Disable Featured Image replacing header image" option in Graphene Options > Display > Header Display Options.
Thanks that worked, but the slider does not contain an image
Did you set a featured image for each post in the slider? Did you set the slider option to display featured image in Graphene Options > Slider Options?
I had it set to first image in post
Then it will display the first image uploaded for that post, not necessarily the first image you insert into the post's content.
